Applying for a
food license
Please make note of the following information and procedures regarding applying for a food license. The below information is a guide to the information you will need when filling out the application form. You are required to fill out the application (PDF), providing the information requested and acknowledging your understanding and acceptance of the information below.
Required documents for a food license application
- An application form:
- A state-required form regarding MN tax identification and Workers’ Compensation:
- Minnesota Business Tax Identification form combined with Workers’ Compensation Insurance form
- An information sheet on how food license classifications are determined—the classification must be confirmed by Environmental Health staff:
If you are a new owner of a previously food-licensed business, follow the directions in #4 below, and skip #5. However, if you are starting a new food establishment, skip #4 below, and go directly to #5.
- You are required to call Environmental Health any time a food licensed establishment changes ownership. No form attached:

- You are required to call Environmental Health any time a new food facility is planned.
Procedure
If you will be a new business establishment, your first step, after receiving this packet of information, is to read and follow #5 above. Do not submit the Application for Food License (#1 & 2) until the Environmental Health Division has approved the construction/layout of the establishment.
For all other applicants, the Environmental Health Division’s staff must confirm or tell you what classification your food license is to be (and the related fee payment)—and to enter this on the application form—952.939.8272. Complete forms #1 & #2 above, and return with fee payment to:

Your license will be prepared within seven (7) working days. The Environmental Health Division will normally hand-deliver the license to you—when they give you their final approval/inspection.
