Please make note of the following information and procedures regarding the customer complaint form. Below is a guide to the information you will need when filling out the customer complaint (PDF) form.

Customers are encouraged to print as they fill out the following information, beginning with today’s date.

Customer contact information

  • Evening phone number

Suspect meal

  • Date eaten
  • Time eaten (a.m. or p.m.)
  • Meal
  • Ingredients

Illness

  • Onset date
  • Onset time (a.m. or p.m.)
  • Symptom (check all that apply)
    • Nausea
    • Vomiting
    • Diarrhea
    • Cramps
    • Fever
    • Other

  • Ill (yes or no)
